The GHWIC program provides culturally relevant Type 2 diabetes prevention education to the Pojoaque, New Mexico community. Through engaging, community-centered activities, the program supports healthy lifestyles and long-term wellness.
Chronic Disease Prevention: Our program provides the community with education and activities to prevent and manage chronic disease, especially, through healthy eating, physical activity, and mental wellness.
Diabetes Prevention and Management: Our program shares information on diabetes screening with local healthcare providers and hosting diabetes management classes through Lifestyle Coach classes. We collect data on community risk for diabetes and community members’ efforts to improve their health.
